CANCELLATION POLICY

A cancelled appointment delays our work. 

When you must cancel, please give me at least 24 hours notice.  I am rarely able to fill a cancelled session unless I know at least 24 hours in advance.  If you are unable to provide at least 24 hours notice when you cancel, you will be charged $100 for your missed session or No Show.
 
Insurance does not cover this fee and you are responsible to pay out of pocket.  The only time I will waive this fee is in the event of serious or contagious illness or emergency.
 

 
PRIVATE PAY
 
I accept major credit cards (Visa, Mastercard, Discover), cash or check for private pay and co-payments.  Payment for services is due at the time of service.  You will receive a Good Faith Estimate of primary and supplemental services as part of your intake paperwork, which you should review prior to your first appointment.  The Good Faith Estimate is not a contract and does not obligate the client to obtain services at this practice.  Note, there may be additional services recommended as part of the course of care that are not reflected in the estimate.  Actual charges may differ from the estimate.  I will provide a receipt for all services, which you may submit to your insurance carrier for reimbursement.  

 

The client/representative may initiate a dispute resolution process if the actual billed charges are substantially in excess of the Good Faith estimate.  Any dispute of charges should be submitted to the U.S. Department of Health and Human Services (HHS) in writing.
